Local Affiliate – non-registered and/or non-practicing individuals who have obtained

a degree in Speech Pathology prior to the implementation of RA11429.

Eligibility

• He/she should be a graduate of BS Speech Pathology or BS Speech

  Language Pathology from an accredited university.

Required Documents

The following are the required documents necessary for the application of Local

Affiliate Member:

• Accomplished Membership Information Sheet on the Website

• Copy of Diplomas received (college and post-graduate)

• Copy of deposit slip or proof of transfer

• Copy of valid Certificate of Registration and/or PRC ID (optional)
